Self Pay Patients
We provide many options to help patients afford infertility diagnostic and treatment services. Typically, our services are paid for in one or more of the following ways:
Our billing staff evaluates each patient’s situation and works with the insurance company as applicable. Many insurance companies in the Utah area cover the initial evaluation and diagnostic testing. We bill the insurance company for services when appropriate. Regretfully, many insurance carriers do not cover infertility treatments such as intrauterine insemination (IUI)or in-vitro fertilization (IVF).
Our staff works closely with each patient to determine the best payment options. There are differences in the insurance preferred provider status for each of the Reproductive Care companies based upon the services they provide. Insurance assistance can also be obtained through a service called Fertility LifeLines. They will call the insurance company to verify benefits for both infertility treatments and medications. Call 1-866-Lets Try or 1-866-538-7879; Monday - Friday 8 am - 12 am EST and Saturday and Sunday 8 am - 6 PM EST.
Reproductive Care Associates, PC (RCA - Tax ID 87-0558673) is the preferred provider for the following insurance companies.

Reproductive Care Center minimizes the cost of the initial consult for patients that have insurance where RCA infertility specialists are not preferred providers, such as SelectMed or Altius. The nurses carefully coordinate testing (hormones, ultrasound, HSG) when possible through the insurance preferred provider testing system in order to minimize out of pocket expense.
Semen analysis with Kruger strict morphology testing can be done at Alta View Hospital (through collaboration with Reproductive Care Center). RCA usually sends lab tests to LabCorp which is a preferred provider for almost all insurance companies in Utah. Insurance companies such as SelectMed and Altius currently do not cover the cost of fertility treatment but Reproductive Care Center provides significant discounts for patients that are self pay for their own treatment.
Insurance Notice about Tri- Care and Tri-West
Please note that all Tri-Care and Tri-West patients must have a referral on file before an appointment will be scheduled. Please have the appropriate form completed and faxed into our office. Lab tests and other procedures recommended usually also require pre-authorization. See the Tri-West Website.
Please check your fertility benefits with your insurance provider as plans vary widely in their coverage. Most insurance companies provide coverage for the infertility evaluation but many do not provide coverage for therapy.
If we are not a preferred provider for your insurance company, please contact the billing office regarding our efforts to add your insurance carrier. We will make every effort to become a preferred provider with your insurance company.

RCC is a preferred provider with Aetna, Blue Cross/Blue Shield, Cigna and United Healthcare for these advanced services. We are currently evaluating other insurance company’s reimbursement for these advanced services and hope to be able to provide more insurance options in the future. If your plan covers advanced services like IVF, and we are not a preferred provider, please contact our business office to evaluate your options.
Fee for Service
The Fee for Service” payment option is available for all of our services. Payment is expected at the time services are provided. We accept cash, money orders, checks with current information, and credit cards. A prepaid cash discount is available for many services (often up to 40% off).
We accept checks, VISA, Master Card, Discover, American Express, and HSA debit cards. We offer a discount for cash payments.
Infertility treatment outcomes are rarely guaranteed (except in programs like our 100% Money Back IVF Guarantee) and thus, as a general rule, we discourage patients from incurring heavy debt to pay for infertility treatments. However, we realize that having children or expanding families is a high priority for many of our patients. For those that believe financing is their best option, we have applications available and can help facilitate the process.
Reproductive Care Finance, PC has several financing options for patients requiring IVF. Up to 100% financing is available for qualified patients purchasing a multiple cycle discount or guaranteed refund program package. Up to 80% financing is available for single IVF cycle plans.
Other Finance Companies
The following information is from CapexMD and Prosper HealthCare Lenders, providers of financing for infertility services. We don’t recommend any specific company; however, many of our patients have had good experiences working with these companies. One advantage of some companies is that they provide financing for fertility drugs.

FP and FSA accounts are benefit package options developed to provide flexibility and choice with coverage and spending on health care including infertility treatment. These plans are typically available to all permanent employees of a company. Most companies permit a review of benefit choices each year to ensure that they continue to meet the employee’s circumstances. These plans can be very attractive because they are tax sheltered. The FSA dollars are withdrawn from the paycheck before taxes. For more information see IRS code IRC §105 and §213 or the company's human resources department.
Patients who have a combined gross income less than $60,000 can apply for a graduated discount on services
The International Council on Infertility Information Dissemination (INCIID) sponsors this program. All taxpaying United States residents who meet the criteria are eligible to apply. You must be a registered INCIID Community member at the Bronze or higher level (approximately $55 annually). There must be a documented need for the procedure. Preference is given to patients with no living children. There must be documented financial need (usually combined income of less than $60,000). Applicants can have no insurance coverage for IVF.
